This completes the setup. In addition, in "Filter", add the following chairman email list settings.

The "Filter name" should be easy to understand when viewed later.
For "Filter Type", select "Custom" and "Advanced".
For "Field A," select "Host Name" and enter "(.*)"
For "Field B," select "Request URI" and enter "(.*)"
For "Destination", select "Request URI" and enter "$A1$B1"
The filter settings have the following meaning:

Apply the request URI to field B
Apply the request URI as $A1 (field A) $B1 (field B)
